Parental Consent

Release and Authorization Agreement

I understand, consent to and agree that Whitworth University and the Center for Gifted Education may photograph and/or videotape my child while he/she is engaging in university and/or the Center for Gifted Education's activities, and that the university and/or the Center may utilize said photographs and video in their discretion, including, but not limited to, use in university publications and other public venues, and that Whitworth University shall own and retain all rights to said photographs/videos.

Medical Assistance Authorization: As the Parent/Guardian, I certify that my son/daughter/ward has my permission to participate in Camp Metamorphosis and that he/she will be subject to the regulations of Whitworth University and Camp Metamorphosis.

I hereby give my consent to the camp staff to attend to any health problems or injury my child may incur while attending this camp. Furthermore, I give my consent for my child's medical treatment and my permission to the attending physician, while understanding that I accept full responsibility for the cost of any medical or other charges in connection with my child's attendance at camp.

Refund Policy:
A full refund will be given upon request up to two weeks prior to the start of camp. A partial refund equaling half of the registration fee will be given upon request up to one week prior to the start of camp. No refunds will be given within seven days prior to the start of camp.
